When I mentor another photographer, they have specific things they want to learn which requires specific types of models:  a family, maternity or senior...etc.  By you filling out this form, you're letting me  know you are down for being a model for a mentoring  session and if you fit the needs for one of my mentors then I will contact you!   Leave your instagram or upload a recent photo to help me decide if you're a good fit for a specific model session!
